Authorized/affected persons
2. Authorized personnel shall understand and be able to implement all aspects of the locking/listing procedure. Affected persons shall understand and respect Lockout tagout and shall not attempt or move Lockout tagout used by others.
Effective training
3. Understanding of locking responsibilities, procedures, methods and requirements comes from effective Lockout tagout training. The knowledge gained through training is embodied through field/operational practice.
The right tool
6.A special set of tools is required, including key, lock, multi-lock locking device, red tag and shift tag.
Alternative methods
7. Complete Lockout Tagout is always the first choice. The establishment of alternative methods must be based on risk assessment of machines, equipment, processes and circuits.
The risk assessment
8. Risk assessment is used to discover the safest possible conditions for an individual operation. Risk assessment must include the identification and implementation of control measures so that other regulatory requirements can be met.
Shift or personnel change
9. The maximum allowed time for each Lockout tagout is the shorter of one shift or the end of the task. It is important to ensure the integrity of the Lockout tagout protocol by using direct Lockout tagout handoff, transition locks, or other appropriate means.
LOTO for contractual actions
10. Company’s top down: An authorized company representative is designated to perform the Lockout tagout procedure. At this time, external service personnel or contractors shall attach their own Lockout tagout to the same energy removal device already locked by company’s representative and fix it.
